Leucoderma or Vitiligo
Vitiligo is more commonly known as leucoderma, which simply means white (leuco) skin (derma), i.e. a disorder where the skin loses its normal coloration. It is also medically known asachromia, which means loss of color.
Basically, Vitiligo can hardly be called as a disease but a skin-disorder that has more social than medical significance, especially amongst the dark skinned people. Due to destruction of the melanin (pigment) cells due to lesser known processes (largely what is called as an auto-immune disorder) the normal skin starts loosing pigments from various parts of the skin, in a varying speed and extent. Our clinical experience based on the treatment of over 2000 cases, suggests that there is a strong genetic factor in the background of most cases, especially those who have extensive vitiligo or those who have vitiligo affecting the finger-tips, toes, lips or the genitals. The indication of strong genetic factor is observed in the form of family history of one or more of the auto-immune diseases such as vitiligo, diabetes, hypothyroid, alopecia areata, cancer, rheumatoid arthritis or allergies.
The vitiligo sufferers are observed all over the world, including the white skin communities. However, epidemiologically most cases are recorded in India (8.8%) and Mexico. Estimated 1-2% of the Americans have vitiligo as per the survey made by the American Academy of Dermatology. Males and females are affected equally, inclusive of children age group. It may begin at any age. Childhood vitiligo is not uncommon.
Leucoderma(vitiligo) Causes
As mentioned above, the reason for leucoderma is still not very clear. However some theories are present and are as follows The first and most supported theory is autoimmune. In this case, the bodies self defense mechanism mistakes the melanocytes as foreign particles and destroys them thus putting melanin production to a halt. There is evidence that points towards this theory and tests are being carried out to determine if this is accurate and how to stop this.
The second theory suggests that the oxidant-antioxidant system of the body is, for an unknown reason, disturbed resulting in loss of melanocytes. A third theory is more based on statistical data than research. Statistics suggest that if a parent has leucoderma, his/her child will have a higher risk of having leucoderma than the rest of the population. So this theory suggests that a genetic factor is present in the causes of this disease. A fourth theory suggests that leucoderma is a direct cause of nerve damage. This can also be true as in a type of leucoderma called as segmental vitiligo, the areas of skin affected are often the areas in contact with the dorsal root of the spinal chord.
Leucoderma(vitiligo) Symptoms
The disease with white patches on the skin surface without other changes is commonly known as vitiligo or Leucoderma. It is also called white spot disease or Leucoderma in which the skin loses its pigment due to the destruction of melanocytes.
Leucoderma symptoms are clearer in darker skinned people because of the contrast of white patches against dark skin. There is a sudden or gradual appearance of Leucoderma with flat areas of normal-feeling skin with complete pigment loss. Lesions appear as flat areas with no pigment and with a darker border.
Leucoderma mostly affects areas of injury particularly the face, upper chest, hands and groin. It may be widespread, affecting both sides of the body. How is Leucoderma Diagnosed? The clinical examination of a patient with acquired, milk-white spots in typical sites is usually based diagnosis of Leucoderma it is usually patterned and symmetrical.
When a health professional assumes that a person suffer from Leucoderma (vitiligo), he/she instigates inquiring the person concerning his or her medical history. Medical history of a person includes a history of vitiligo in family, sunburn, or other skin sufferings of vitiligo before depigmentation started.
